You need to

• Read the project information sheet.
• Decide if you want to sign up. You don’t have to sign up. It’s your choice.
• Sign the consent form. This says you agree to sign up.
• Tell us who your GP is. We’ll then ask them to sign up to the study too.

There might not be a direct benefit to you for signing up.

You will get to help a project that is working to make going to the GP better for people with intellectual disability.

We will ask you to answer a survey. 

It will ask things like your age, where you live and about your health.

You can answer the survey over the phone, on the computer or we can post it to you.

We will ask you if you want to talk to us after you have a health check with your GP practice. 

You can say yes or no.

We will ask you questions about how the appointment went.
